Are you ready to join? Please fill out and bring the completed form for us to register your child. Please remember to bring your child's birth certificate and health card as we cannot register your child until we have these documents.
Furthermore, if you are aware of any medical conditions (including allergies) that your child has, please also fill out and bring a completed Detailed Health Questionnaire (DND 2570).

Before starting your online application ensure you have your cadets health card ready! Once you begin your online registration you will be unable to leave the session without having to restart.
Step 1 - go to registration.cadets.gc.ca/get-started.html and look for "45 Jervis Bay Royal Canadian Sea Cadet Corps" and type in your email address. Step 2 - follow the link provided to your email (make sure to check your junk folder!) and complete the form and submit! Step 3 - our staff at 45 Jervis Bay will recieve your information and will get in contact with you as soon as possible! |
Cadet Code of Conduct
Every year, cadets will be required to complete the most current Code of Conduct. This is to ensure all cadets are aware of what we, the officers and staff of Regional Cadet Support Unit (NW), expect from all cadets. Please complete the following form and email it to 45sea@cadets.gc.ca

Section 9 – Tick yes or no if you wish your child participates in our program, sign and date.
After completing this paperwork, please email us a copy of the registration form. If your cadet requires a DND 2570 (DHQ) please let the staff of Jervis Bay know and we will provide you with pre-paid postage and ask that you *mail the DND 2570 (DHQ) to the following address:

*Having parents send in their cadet's DND 2570 (DHQ) is to ensure that your cadets personal medical information remains confidential. If you have any concerns, please speak to an officer or email us at 45sea@cadets.gc.ca

- Section 3 – Fill in all boxes
- Section 4 – Saskatchewan Health Card is preferred, but simply tick public and provide your account number
- Section 5 – Simply tick yes or no
- Section 6 – Provide your details
